Post-Acute Care/Rehabilitation Services

Fellowship Manor has also achieved Post-Acute Care Certification from the Joint Commission. Our nursing staff is specially trained to provide clinical care that meets the Joint Commission standards. The rehabilitation staff is highly skilled in physical, occupational and speech therapies that ensure patients reach the highest level of function in the shortest possible time. Personalized support, state-of-the-art equipment and a high staff-to-resident ratio encourage the physical, emotional and spiritual well-being of each resident.

PHYSICAL THERAPY:  Physical Therapy will provide outcomes-oriented interventions to ensure each patient reaches the highest functional level and optimal quality of life. Our Physical Therapy treatment will restore physical independence utilizing evidence based practices.  The Physical Therapy Staff will provide individualized care for the patient to promote increased strength and coordination, range of motion, and balance while also using a variety of ways to alleviate pain.

OCCUPATIONAL THERAPY: Occupational Therapy is primarily responsible with improving, developing or restoring the sensory, motor, cognitive, perceptual and psychosocial skills which have been impaired by illness or injury.  Patients are evaluated by the Occupational Therapist and an individual treatment plan is developed. Occupational Therapy treatment will focus on increasing function through the use of therapeutic activities to increase self-care abilities, joint motion, muscle strength and coordination, enabling the patient to overcome physical, psychological, cognitive and/or visual limitations through the selective use of assistive devices, and compensatory techniques. Occupational Therapy treatment will also assist the patient to achieve his/her optimal level of independent functioning in activities of daily living.

SPEECH THERAPY: The Speech Language Pathologist will evaluate and design a therapy program to improve speech, language, and swallowing functions to increase functional communication skills that include auditory comprehension, speech production, expressive language skills, cognitive/memory abilities and non-verbal communication. The Swallowing therapies are designed to establish safe and efficient swallowing of liquids and solid foods to meet nutritional needs.

Regardless of what Rehabilitation Services are needed, our team will meet the needs of each patient to ensure increased quality of independence and function.
